– Legalized Declaration of Origin stating the Active Ingredient for class 5 applications
– Complete passport copy of the signatory
– Certified copy of priority document, if priority is claimed
Renewal
– Power of attorney duly notarized and legalized from the Iraqi consulate
– Copy of trademark registration certificate
Assignment
– Power of attorney from the Assignee duly notarized and legalized from the Iraqi consulate
– Assignment Deed signed by both parties duly notarized and legalized from the Iraqi consulate
– Complete passport copies of the signatories
– Copy of trademark registration certificate
Change of Name / Address
– Power of attorney stating the new name / address duly notarized and legalized from the Iraqi consulate
– Change of Name / Address document duly legalized from the Iraqi consulate
